What Type of Amnesia would this be?

I'm writing a story in which the main character, a girl 11 years old, falls from a certain undetermined height and is swept down a river. She smacks her head around quite a bit, and now cannot remember her last name, her phone number, or the names of her brothers (save for one). This amnesia can be reversed, as later on in the story, she has a flash as everything comes back to her. If you could help me figure out what this type of amnesia would be, I'd be very grateful! Thanks much! What Type of Amnesia would this be?
It is a post-traumatic amnesia.

Amnesia is a condition in which memory is disturbed or lost. The causes of amnesia have traditionally been divided into categories. Functional causes are psychological factors, such as mental disorder, post-traumatic stress or, in psychoanalytic terms, defense mechanisms. Amnesia may also appear as spontaneous episodes, in the case of transient global amnesia

There are 2 forms:

* In anterograde amnesia, the ability to form new memories through memorization is impaired or lost. A person may find themselves constantly forgetting information, people or events after a few seconds or minutes, because the data does not transfer successfully from their conscious short-term memory into permanent long-term memory

* In retrograde amnesia, a person's pre-existing memories are lost to conscious recollection, beyond an ordinary degree of forgetfulness. The person may be able to memorize new things that occur after the onset of amnesia (unlike in anterograde amnesia), but is unable to recall some or all of their life or identity prior to the onset

She belongs to retrograde amnesia. What Type of Amnesia would this be?
